But our healthcare system isn’t designed to deliver evidence-based interventions that actually prevent dementia. This care is incredibly time consuming and requires a team of providers, clinical staff, and support staff who are trained in prevention, lifestyle medicine, cognitive care modalities, and health system navigation.

You want to help these patients, but you’re likely not given the time or resources needed to provide high-touch, wraparound care.

That’s why we’re here. Mindful Integrative Medicine is a team-based dementia prevention and cognitive care clinic led by primary care physicians.

We exist to partner with Central Pennsylvania’s primary care and neurology providers so that patients who are at risk for - or in the early stages of - cognitive decline can receive timely, personalize team-based care to protect and preserve their cognitive function.

We don’t want to replace you or the important role of the primary care provider - we want to fill the gaps that exist in our current healthcare system so that patients get the care they need.

  • Our process is simple:

    1. Evaluate and identify each patient’s current cognitive function and risk factors for cognitive decline or dementia.

    2. Develop an individualized plan to prevent - or treat - cognitive impairment.

    3. Support the brain health journey through health coaching, chronic care management, health system navigation, caregiver training and support, and coordination across the interdisciplinary care team.

    4. If appropriate, streamline evaluation and referral for disease modifying therapy.

  • You see patients every day who would benefit from extra support of their brain health, including those who are:

    • worried about cognitive changes in themselves or a loved one

    • are at elevated risk for cognitive impairment but do not yet exhibit cognitive changes

    • have diagnosed mild cognitive impairment or dementia

    • need a referral to neurology for cognitive concerns

    • are the caregiver of a person living with dementia

    We provide access to neuropsychological testing and domain-specific cognitive evaluation within days, not weeks or months. This increases the likelihood that cognitive impairment is identified before patients experience irreversible functional decline.

  • Our model is designed around these guiding principles:

    1. Access and Equity: We’ve enrolled as primary care providers with Medicaid and Medicare so that we can increase access to prevention and cognitive care. More information about the payers we work with is available here.

    2. Team-Based Care: Patients need more than diagnosis and treatment - they need emotional support, education, lifestyle change, and more. We supply or coordinate with allied health and community resources to ensure these needs are met under the physician-directed care plan.

    3. Integrative Approach: Brain health and mind health are inextricably linked, so while we use evidence-based Western medicine, we also embrace complimentary modalities to provide whole-person care.
